Purpose: To act on service-related complaints

Monday to Friday – 8:00 a.m. to 5:00 p.m.

Get a Client Feedback Form from the Guard-on-Duty at the Public Assistance and Complaints Desk

Fill-up the Feedback Form

Give Feedback Form to a client.

Instruct the client

 Drop the filled-form at the Suggestion Box

or

Submit the Form to the Records Officer

Stamp receive the submitted form and record 3 mins  Records Officer
